I concur with Jorgen. The risks are minimal and I have never had or ever heard of issue extending the schema for ConfigMgr. The few changes made to the schema are completely dedicated and unique to ConfigMgr and do not affect anything else.Jason | http://blog.configmgrftw.com | Twitter @JasonSandys

I agree with Jörgen and Jason.
We began extending the AD Schema in SMS2003 RTM, and since then, a fair few years ago, I've only had one story related to extending the schema and "issues" experienced, and this single instance came from a company that couldn't administer itself out of a wet paper bag, meaning their AD was in a complete mess and changing the Schema meant some DC's had the changes whereas others did not. As long as AD is healthy there is no way on this earth that extending the Schema can break a companies directory services (AD). Just make sure your AD is healthy, that there are not existing issues present, typical advise for any product really.
