Configuring TMG Server to allow SIP softphone traffic

Dears,
I configured my TMG server to allow zoiper softphone to make SIP calls but i have a problem when i tried to initiate the call the end user hear me and i can hear him after 28 second from starting the call, but if the end user initiate the call from his side he can hear me and i can hear him from starting the call.
Please help me to avoid this issue with correct configuration.
