Sharepoint Workflow Designer - Third workflow step not firing - COMPLETELY STUCK

Hi all,
Hope you can help :-) With All workflow start options for items ticked (Automatically start when new item is created \ changed etc.) the third condition is not firing in my workflow. The workflow looks like this (on a custom form)...
1) Form created - send email to tester
2) Tester approves - send email to DBA
3) DBA approves - send email to user
Now the 1) and 2) steps fire without a problem the 3) doesn't work at all even (doesn't appear to fire) though the logic is very very simple. Any idea why this would happen? I'm using Sharepoint designer 2007.
