    is it possible to log in to SQL Server 2008 or 2012 on Windows 2008 R2 using a DOD-issued CAC?  If so, do you have any references on how to do this?

    SQL server support only SQL & windows authetication.

    To use CAC access for SQL server you need to look for application devlopers who can pass you CAC credentails to SQL server as a login.
