How can I get Multiple alerts for a Web Application Monitor

I'm monitoring a web page for login response time with a web application monitor and I'd like the monitor to send an alert every five minutes as long as the response time exceeds 5 secs. But I can only get the monitor to send one alert and then I need to manully reset the health.
What's the trick to getting continuous alerts?

Thanks for the response. However this doesn't explain how to have a monitor send continuouis alerts untill a condition is returned to normal.
