Windows Server 2003 to 2008 Migration

I'm in the process of migrating Windows Server 2003 Enterprise (DC and file/print) to new HP Proliant hardware Windows Server 2008 Standard. What's the best practice for properly moving and upgrading? I was hoping to find instructions and/or checklist to ensure I don't miss any steps.
