How do i setup email alerts with user name when they are locked out.

I have task scheduler working. It send me alerts but the alert only shows someone was locked out. How do I work it where it shows me who was locked out. I need user name so I can unlock and track potential problems of users who get locked out alot.
Using Windows 2008. Have a powershell script running in when the event happens.
