Can you explain little more on this, do you want to create Hyperlink for what?

Hi ,
I understand that you want to create a hyperlink in a survey .You can achieve this by using SharePoint Designer .
1. Open the site in SharePoint Designer .Open the NewItem.aspx page .
2. Choose ‘show design view ‘ .Delete the web part on the page .
3. Choose Insert>SharePoint Controls>custom list form .Set ‘the list or document library to use for form’ as survey . Choose ‘Type of form to create’ as new item form .
4. Click Ok to insert the custom list form .
5. Put the mouse curse to the column where you want to add hyperlinks .Then choose insert>hyperlinks .
6. After you have accomplished this, save the page and check what it looks like now by responds to the survey .

I am trying to create a hyper link in sharepoint 2003 survey.
I followed all your above steps and able to add hyper link in Newitem.aspx page.
Once i done editing with newitem.aspx page do i need to publish this page??
For testing when i run on browser iam getting below error :
An unexpected error has occurred.
Web Parts Maintenance Page: If you have permission, you can use this page to temporarily close Web Parts or remove personal settings. For more information, contact your site administrator. -
