sent items are still in outbox

We just noticed a starnge thing after installing Update Rollup 1 for Exchange Server 2010 Service Pack 2.
Some of the users send emails fine and emails get delivered but the emails get stuck in the outbox and stay there until someone move them from the outbox to the sent box.
Any idea what might be causing this issue and how to solve it? People are freaking out that their emails are not being sent out.

Does that happen in OWA
do they have delay send rule configured in outlook?

Do we have any Antivirus or 3rd party Software acting on the Client / Server - Can we get this isolated.

OWA was the same, sent items were not in the sent folder till they were manually moved there from the outbox.
We have outlook 2010 with the latest patches applied.

No the issue was not due to antivirus. Emails get sent out fine but not moved from the outbox to the sent folder.
The users had cached profiles so we disabled that and now we don't see that issue and no user noticed it either.

There is no delay rule setup and OWA showed the same thing. In OWA we can't see the outbox.
Use OWA Light to see the Outbox.
