Lync 2010 Multi Tenancy Support

Just browsed through the "Microsoft Lync Server 2010 Multitenant Pack for Partner Hosting Deployment Guide" but I am not to clear on how the PABX integration works in this scenario. Is there anybody who has experience for this type deployment and is willing to share some thought on it?
How does the multitenant Lync client utilize a PSTN gateway - is this something the service provider has to have at the hosting location? Could the multitenant pack be a replacement for on premise PABX and related services or is there still a PABX/Gateway needed on the customer site to provide telephony services through the Lync 2010 client?
Furthermore - it looks like the Lync 2010 multitenant pack requires a hosted Exchange 2010 setup?
Also how does this makes sense of a business model?
