Sharepoint Survey - I can't view all responses and now some users in the members group don't have access to save & submit the survey

The problems right now are:
- I can’t view all survey responses – I should be able to since I have manage list permissions by virtue of being an Owner and having Full Control
- Some authenticated users in my Visitors group can’t save and submit their survey responses – I don’t know if something happened and this now applies to everyone. When they answer questions and then try to save and submit the survey, they receive a message Error: Access Denied and an option to sign in as a different user or request access. When access is requested and an automatic email is sent to them granting access, they still receive the same error message.
I set up a survey that I would like all employees on our domain (all authenticated users) to be able to access without seeking additional permissions. I want them to only view their own response, and no one else's. However, I need to be able to see all responses so that I can analyze the survey data.
I can't think of what is wrong. The permissions seem to be correct for both the site and the survey item.
Under the Site Settings, there are 3 groups:
Owners - Full Control - myself and our system admin
Members - Contribute - myself and our system admin
Visitors - Contribute - all authenticated users on our domain
Under the Survey Settings -> Advanced Settings, I specified users can read only their own responses and edit only their own responses. The instructions on this page say that Note: Users with the Manage Lists permission can read and edit all responses. Learn about managing permission settings.
As an owner of the site with full control, I should be able to view all survey responses, but I can only see my own response.
Does anyone have an idea for how to fix this and the other problem that authenticated users do not h ave the contribute access they seem to have? Is it something to do with errors in how our sharepoint is set up, or how it is tied to our active directory? Thank you so much,

First of all I'm not sure why your visitors group has contribute permissions when you already have a Members group with contribute permissions defined. Visitors is relegated to 'Readers'.
Second, check the survey's permissions to see if it is inheriting from the parent site. There's no way to tell how your AD is set up, but so long as SP is seeing the users and adding them you should be ok on that front.

Thanks so much for responding!
So I deleted the visitors group, and added all users on our domain to the Members group with Contribute permissions. Still, all users are receiving the Error: Access Denied when they try to answer and save the survey.
I had the system administrator answer the survey, and I can now see his responses and my own. So there is something he has that the rest of my authenticated user group needs to have.
I did check the permissions for the site and it is just the owners and members now. How can I tell if it's inheriting from the parent site.
Please let me know if have more ideas & advice!!

Look at the survey's settings > permissions for this survey and that will show you what permissions the survey has.
If you have a button in the ribbon under Inheritance that says 'Inherit Permissions' then you're using unique permissions.
My guess is that your survey isn't correctly inheriting permissions of the parent. You can also add NTAuthority\Authenticated Users to the contribute group to ensure you have everyone covered.

OK, I set the survey to inherit the permissions of the parent site, and the permissions for the parent are:
Owners - Full Control - myself and our system admin
Members - all authenticated users
But my problems are still:
Still, all users are receiving the Error: Access Denied when they try to answer and save the survey. A week ago, they were able to save and access the survey.
I cannot see the survey responses from anyone except the system admin and myself. I can only see 2 of 14 responses.
Any ideas and help is so appreciated.
