  • Hi Leili vazietan,

To add multiple radio buttons in an InfoPath form, please do as following:

    1. Add a Drop-Down List Box control to the custom form. Click “Enter choice manually” and then add the custom options to the control.

    2. Right click on the Drop-Down List Box control and select “Options Button” via “Change Control”.

    3. Duplicate the Option Button as many times as you want. Then manually type the option values next to each button.

    4. Right click on each Option Button and go to "Option Button Properties...", change the value for the current option under "Value when selected". Click “OK” to save.

  • Hi Leili vazietan,

I’m afraid it is not supported to change the radio button icon.

As a workaround, you can put the Correct/Incorrect Options as texts in the field “Value when selected” so that users can see what the options represent.
