  • Hi,

    this means that you have any application which caused a ACCESS VIOLATION.

    Try to deinstall all unneeded software. Get autoruns from sysinternals and prove which software is loaded at startup and remove unnecessary points.

    Try to find out where "safe_url.dll_unloaded" lies. Prove it with VirusTotal --> Link

    Try to get a dumpfile from the explorer.exe HowTo

    Try to analyze this dumpfile with WinDBG - normally the cause can be seen here.

    You could also start ProcMon from the Sysinternals Tools and reproducing the error. Afterwards stop grabbing the events in Procmon and filter them. Look for ACCESS DENIED Status and take also a look at this nice example which will help you: Link

    Lots of steps but they will hopefully help you a little to isolate the problem.
