QR Code has expired after forcing user to redo MFA credentials RRS feed

  • I manage the Microsoft accounts for my company using Azure Active Directory and MFA is enforced by our policy. So I have a user experiencing issues with the Authenticator app. When they logged in, no notification was sent to his app to approve the login, even though it was asking for approval.

    So I tried resetting the users MFA devices and have them provide the MFA device again. While setting up the device, we scanned the QR code and it comes back with "QR Code has been used, please regenerate the QR code. Also ensure notifications are enabled on the device." We even tried adding an Additional Security Device (to regenerate the code) through there profile but it is not letting us in until an MFA device is provided. However, We can get access to his account when the MFA is disabled on the account but we can't setup the Additional Security Device. Not sure how to get the MFA working correctly again. Let me know of any ways to get this to work. Thanks.
