Customizing incident form

By customer needs, I need to modify the form of incidents and add some fields in self-service portal.
In version 2010 I did it without problems because it was released the source code.

How I can do it in version 2012?

When is going to release the source code?

Hi,

you don't need the source code anymore as you can now create/customize service request offerings and a service catalog in the UI. It lets you define user prompts and you can map each prompt to the resulting work item.
