I am using exchange 2010 & Lync 2010,i would like to provision exchange Mailboxes from FIM 2010 R2 Portal.
What are the perquisites to start this process. we have one mailbox database in exchange.
What are the configuration need to take, what types of attributes need to create in outbound sync rule or is there any permission required for any account.
If any one can help I would thankful to them.

To (de)provision the Exchange/Lync objects you can use the AD MA.
Another way to (de)provision mailboxes, is for example by using a powershell MA that executes the Exchange PoSH CMDlets. Soren Granfelt has one for free.

You need to check the Event Log in the FIM Sync Box to see what the ma-extension-error is about? I suspect that the ADMA account does not have proper Exchange permissions to run the 'Update-Recipient' cmdlet (which is what it does behind the scenes).
Could you verify that the account specified for you AD MA has enough Exchange permissions? Also, verify that the FIM sync box can connect using remote PowerShell to your Exchange CAS servers - otherwise you'll have problems as well.
The Exchange errors that you're seeing is most likely related to the Exchange CMDlets not be runned properly against the new Exchange mail.
