What setup option to choose when I want to install SCCM 2012 on a new server, and want to upgrade from another SCCM 2007 server.
a- Install a configuration manager Primary Site
b- Install a Configuration Manager Central Administration site

My first gues was right. I should have choosen a-. When chosing b- it's like only for administration and reporting purposes.
choose a- to also install a primary site with distribution site management point and so on.

But Do I have to join the primary site to an excisting hierarchy or Install the primary site as a stand-alone?

The only path from ConfigMgr 2007 to ConfigMgr 2012 is a side-by-side migration. There is no joining a ConfigMgr 2007 site to a 2012 site or vice-versa -- they are two very different beasts from a hierarchy perspective.
ConfigMgr 2012 includes a complete migration toolkit to migrate from 2007.
Thus, your choice of a CAS or Primary is dependant on a proper design of your 2012 implementation.
