  On MSDN or Volume Licensing Download site, search for "MDOP" and you'd find MDOP 2009 R2. That one includes App-V Management Server 4.5 SP1.
  
  Also, looking for "SoftGrid" on that location may point you to the right location. Perhaps it leads you to the "Terminal Server / Remote Desktop Services / VDI" download, but the Server Components are the same for TS/RDS and Desktops
