    1. Which Forms are mandatory for a Workflow?
    2. Can we create our own custom Form (5<sup>th</sup> Form) apart from the forms(4 forms) above?
    3. In how many ways we can design the forms (like InfoPath Forms, .aspx Forms) ?

Answers

    • Association and initiation form, these forms are displayed before the workflow starts are used to collect information that apply to a workflow. Association forms are filled by an administrator or power user that assigns the workflow to a share point element, which allows them to specify default settings for this instance of the workflow. 
    • Initiation forms are filled in by the end users once they start the workflow. This allows them to specify default settings for a specific running workflow. 
    • Modification forms - enable the user to modify a running workflow. 
    • Task Form - this can be used to substitute for this task created for the WF.
    • Can we create our own custom Form apart from the forms(4 forms) above?  You can create your own custom tasks forms using VS or InfoPath forms. They would come under custom task forms. 
    • In how many ways we can design the forms (like InfoPath Forms, .aspx Forms) ?  Two ways, using Visual Studio or InfoPath. 
    • Which Forms are mandatory for a Workflow?You could create a simple workflow without using any of these forms. If the requirements are such that you need user input, or creating any custom tasks, you would need to create an initiation form.
