Hi Tech.bird,
The SQL 2000 management pack version 6.0.6648.0 can be installed separately. Please refer to the next link:

Can be downloaded here and works OK under SCOM 2012 R2:

SQL MP 6.1.314.35 removes the support of SQL Server 2000. One of the workaround is downloading previous SQL MP(6.0.6648.0) and then import just SQL Server 2000 (Monitoring) and SQL Server 2000 (discovery) as stated on http://thoughtsonopsmgr.blogspot.dk/2010/08/how-to-monitor-sql-2000-with-latest-sql.html. Moreover, SCOM 2012 R2 agent supports Windows 2003 SP2 platform. This means that your SQL Server should be installed on Windows 2003 SP2 otherwise it is useless to import SQL Server 2000 monitoring or discovering MP. For supporting configuration of SQL Server 2000, please refer to http://technet.microsoft.com/en-us/library/aa175274(v=sql.80).aspx

SQL 2000 is not supported in SCOM 2012, but there is a workaround: http://thoughtsonopsmgr.blogspot.dk/2010/08/how-to-monitor-sql-2000-with-latest-sql.html

SQL 2000 MP - this is not supported and there is no workaround for this. But you can install it and it does work as long a the underlying OS is supported for the SCOM agent.
