    Things done:

    Console Manager:

    - Stop SCOM services then rename Health service state

    - Import updated Management pack for Windows Core sever

    - Port check for both server and client 5723 successfull

    Agent / Client

    - Stop SCOM services then rename Health service state

    Install agent manually and approve on the OM.

    Please let me know for any help.

    Which kind of OS you want to monitor?

    You need to import the corresponding MP.

    System Center Management Pack for Windows Server Operating System

    https://www.microsoft.com/en-in/download/details.aspx?id=9296

    You can import MP from catalog or disk.
