Placement of Custom Monitors and Overrides

I've read that all custom monitors & overrides should be placed in a newly created MP and never in the default MP, which sounds pretty clear cut, and there should be one override MP for every imported MP that is being used. Our environment started out as a test quickly became production about a year ago. The implementer is no longer here and we would like to clean up our structure.
My question is, we create most of our custom monitors under the "Windows Server Operating System" class which is listed as part of the "Health Library" MP. Most of the monitors are either event log monitors or service up/down monitors. Should I have a "Health Library Overrides" MP for the custom monitors that we are creating? Also, how is the "Windows Server Operating System Library" different and should an Override based on that MP instead?
I'm sure this is a very basic question, but I've only been working w/SCOM for a few months and am learning as I go along.

Service monitors, I would target the management pack that the service would reside in. If an Exchange Service, Exchange override or Exchange Custom. If windows server 2008, Windows server 2008 Operating System override/custom. If Windows 2003, windows 2003 operating system override/custom.
This might be a tad bit nit picky, but makes managing your environment much easier. Also, document ALL overrides you do in Excel with the changes made, when it was made, why it was made, and what mp and version it was from/intended for and where it is stored (what override mp).

Not sure why the person targeted that class. Not wise in my honest opinion. If you have time you should work on getting a grasp on all the overrides and work on creating new ones that target the correct classes, store those in their proper override mp's and then get rid of the overrides that were in place before you started.
You can continue to do what you are doing, but I would be more specific in my targeting even for new monitors.

Is there a document or URL that you can recommend which reviews this in-depth? When creating an event log or service up/down monitor, what class would you recommend?
