  I don't have access to iGnite level 400 documents. Can you provide details about what iGnite level 400 document may be?

  Yes, but you should choose a internel private CA, if you plan to install mor roles and a media gateway.

  Yes,you can use self-signed certificate for Lync phone edition,however I don't recommend you deploy it in this way since I have seen someone encountered some authentication issues or prompts,here are some old threads talking about it for your reference.
