I am setting up a rating scale survey in SharePoint 2007. I have a range of 4, but SharePoint only allows the customization of 3 range text boxes (low, medium, high). I need to be able to have a "no value add, minimal productivity gain, moderate productivity
gain, must have" choices. I am already using the N/A field for a "did not utilize".
Is this possible? If so, how do I go about doing it?
