  • Here is a simple example:

    <html>
    	<head>
    		<title>Select Printer Form</title>
    		<style type="text/css">
    			table {border-collapse: collapse;}
    			table td {
    				border: 1px solid black;
    				font-family:serif;
    				font-size:larger;
    				font-weight: bolder;
    			}
    			td { width: 150px;}
    		</style>
    	</head>

    	<body>

    	<H3 align="center">Select a printer
    	<P>&nbsp;</P>
    	<table  align="center">
    	<tr>
    		<td><input type="checkbox" name="CheckBox1" value="printer1"/>printer1</td>
     		<td><input type="checkbox" name="CheckBox2" value="printer2"/>printer2</td>
    	</tr>
    	<tr>
    		<td><input type="checkbox" name="CheckBox3" value="printer1"/>printer3</td>
     		<td><input type="checkbox" name="CheckBox4" value="printer2"/>printer4</td>
    	</tr>
    	<tr>
    		<td><input type="checkbox" name="CheckBox5" value="printer1"/>printer5</td>
     		<td><input type="checkbox" name="CheckBox6" value="printer2"/>printer6</td>
    	</tr>
    	<tr>
    		<td><input type="checkbox" name="CheckBox7" value="printer1">printer7</td>
     		<td><input type="checkbox" name="CheckBox8" value="printer2">printer8</td>
    	</tr>
